Nearly half of all Americans have some form of periodontal (gum) disease. Without proper daily hygiene and treatment, this aggressive disease can ultimately cause tooth loss. It also appears the effects of gum disease reach beyond the mouth, as researchers have found relationships between it and other systemic diseases.
Inflammation, the body’s response to infection, is at the center of these relationships. In the case of gum disease, periodontal tissues become inflamed as the body attempts to isolate and fight the infection. If the inflammation becomes chronic, however, it will begin to damage gum tissues.
Inflammation is also a major feature of diabetes, a condition in which the pancreas doesn’t produce enough insulin. Without enough of this hormone that transforms sugar into usable energy for the body, the sugar accumulates in the blood stream; as a result, the patient becomes more susceptible to an exaggerated inflammatory response when the body encounters an infection. This is especially true for periodontal infections: the resulting inflammation can be greater and harder to control in diabetic patients. What’s more, uncontrolled gum disease may worsen their blood sugar levels.
Although not as prominent as with diabetes, cardiovascular disease also seems to share a connection with gum disease. This collection of chronic conditions like high blood pressure or restricted blood vessel flow raises the risk of heart attack or stroke. Like gum disease, inflammation is a major component in the progression of cardiovascular disease — in fact, both diseases leave similar chemical “markers” in the blood that indicate their early development.
Ongoing research has also produced some promising treatment findings for both gum disease and inflammatory diseases, which also include osteoporosis, respiratory disease and rheumatoid arthritis. We’re now finding in many cases that treating one side of the disease connection can have positive effects on the other side. For example, diabetics who receive professional treatment for gum disease may see better blood sugar control.
With this in mind, the best approach is to practice effective, daily oral hygiene to reduce the risk of gum disease, coupled with regular office cleanings and checkups. Not only will this help you maintain optimum oral health, it may also contribute to better management of other conditions you may have.

Most of us have encountered something hot that’s burned or scalded the inside of our mouth—not a pleasant feeling. But what if you have a similar burning sensation without eating or drinking anything to cause it?
It’s not your imagination: It could be a condition called burning mouth syndrome (BMS), the feeling your mouth is burned or scalded without an apparent cause. It’s often accompanied by dryness, numbness, or tingling. You may feel it throughout the mouth, or just in “hot spots” around the lips, tongue or other mouth structures.
Researchers haven’t pinpointed exact causes yet for BMS. It’s most common in women around menopause, connecting it to a possible hormonal imbalance. It’s also been linked to diabetes, nutritional deficiencies, medication, acid reflux, cancer treatment or psychological issues. Because it can persist for years, BMS can contribute to irritability, anxiety or depression.
If you’re experiencing BMS, there are things you can do to diminish its effect. First, though, have your dentist give you a complete oral exam and take a thorough medical history. They can then give you specific treatment recommendations based on what they reveal.
For example, if symptoms seem to increase after brushing your teeth, you might be having a reaction to a toothpaste ingredient, usually the foaming agent sodium lauryl sulfate. Your dentist may recommend experimenting with other toothpaste brands.
Other treatment options include:
- Alleviating dry mouth symptoms by changing medications (as your doctor advises), drinking more water and using saliva-boosting products;
- Quitting smoking and reducing your consumption of alcohol, coffee and spicy foods;
- Chronicling your diet to look for connections between individual foods and BMS flare-ups—you may need to restrict these in your diet.
- And because it seems to aggravate BMS symptoms, reducing acute stress with relaxation techniques or therapeutic counseling.
If your dentist can’t fully diagnose your condition or the steps you take aren’t reducing your symptoms, you may be referred to an oral pathologist (a dental specialist in mouth diseases). The key is not to give up until you find a workable treatment strategy. Through a little trial and error, you may be able to overcome the discomfort of BMS.

Holistic medicine aims to provide healthcare for the “whole” person. While it's a worthy approach, the term has also been used to advance ideas, including in dentistry, at odds with solid scientific evidence.
Here are 4 “holistic” oral health claims and why you should be wary of them.
Root canals are dangerous. It might be shocking to learn that some claim this routine tooth-saving procedure increases the risk of disease. The claim comes from an early 20th Century belief that leaving a “dead” organ like a root-canaled tooth in the body damages the immune system. The idea, though, has been thoroughly disproved, most recently by a 2013 oral cancer study that found not only no evidence of increased cancer, but an actual decrease in cancer risk following root canal treatment.
X-rays are hazardous. X-rays have improved tooth decay treatment by allowing dentists to detect it at earlier stages. Even so, many advise avoiding X-rays because, as a form of radiation, high levels could damage health. But dentists take great care when x-raying patients, performing them only as needed and at the lowest possible exposure. In fact, people receive less radiation through dental X-rays than from their normal background environment.
Silver fillings are toxic. Known for their strength and stability, dentists have used silver fillings for generations. But now many people are leery of them because it includes mercury, which has been linked to several health problems. Research concludes that there's no cause for alarm, or any need to remove existing fillings: The type of mercury used in amalgam is different from the toxic kind and doesn't pose a health danger.
Fluoride contributes to disease. Nothing has been more beneficial in dental care or more controversial than fluoride. A proven weapon against tooth decay, fluoride has nonetheless been associated with ailments like cancer or Alzheimer's disease. But numerous studies have failed to find any substantial disease link with fluoride except fluorosis, heavy tooth staining due to excess fluoride. Fluorosis, though, doesn't harm the teeth otherwise and is easily prevented by keeping fluoride consumption within acceptable limits.
Each of these supposed “dangers” plays a prominent role in preventing or minimizing dental disease. If you have a concern, please talk with your dentist to get the true facts about them.

The change from primary teeth to permanent is an announcement to the world that a boy or girl is "growing up." "Growing up," though, is still not "grown"—the new teeth are still in a period of development that can affect how we treat them if they're injured or diseased.
While a new tooth erupts with all its anatomical layers, the middle dentin is somewhat thinner than it will be after it matures. The pulp, the tooth's innermost layer, produces new dentin and gradually increases the dentin layer during this early development period. While the pulp continues to produce dentin over a tooth's lifetime, most of it occurs in these early years.
To prevent or stop any infection, we would normally perform a root canal treatment in which we remove the pulp tissue and fill the empty pulp chamber and root canals. This poses no real issue in an older tooth with mature dentin. Removing the pulp from an immature tooth, though, could interrupt dentin development and interfere with the tooth's root growth. Besides a higher risk of discoloration, the tooth could become more brittle and prone to fracture.
That's why we place a high priority on preserving a younger tooth's pulp. Rather than a root canal treatment, we may treat it instead with one of a number of modified techniques that interact less with the pulp. Which of these we use will depend on the extent of the pulp's involvement with the injury or disease.
If it's unexposed, we may use a procedure called indirect pulp therapy, where we remove most of the tooth's damaged dentin but leave some of the harder portion intact next to the pulp to avoid exposure. If, though, some but not all of the pulp is damaged, we may perform a pulpotomy: here we remove the damaged pulp tissue while leaving the healthier portion intact. We may then apply a stimulant substance to encourage more dentin production to seal the exposure.
These and other techniques can help repair an injured young tooth while preserving most or all of its vital pulp. Although we can't always use them, when we can they could give the tooth its best chance for a full life.

Because the mouth is one of the most sensitive areas of the body, we go to great lengths to eliminate pain and discomfort associated with dental work. Anesthesia, both local and general, can achieve this during the actual procedure—but what about afterward while you’re recuperating?
While a few procedures may require prescription opioids or steroids to manage discomfort after a procedure, most patients need only a mild over-the-counter (OTC) pain reliever. There are several brands available from a group of medicines called non-steroidal anti-inflammatory drugs (NSAIDs). NSAIDs like aspirin or ibuprofen work by blocking the release of prostaglandins into the body, which cause inflammation in tissues that have been damaged or injured.
Unlike their stronger counterparts, NSAIDs have fewer side-effects, cost less and aren’t addictive. And unlike opioids NSAIDs don’t impair consciousness, meaning patients can usually resume normal activities more quickly.
But although they’re less dangerous than opioids or steroids, NSAIDs can cause problems if taken at too strong a dose for too long. Its major side effect is interference with the blood’s clotting mechanism, known as “thinning the blood.” If a NSAID is used over a period of weeks, this effect could trigger excessive external and internal bleeding, as well as damage the stomach lining leading to ulcers. Ibuprofen in particular can damage the kidneys over a period of time.
To minimize this risk, adults should take no more than 2400 milligrams of a NSAID daily (less for children) and only for a short period of time unless directed otherwise by a physician. For most patients, a single, 400 milligram dose of ibuprofen can safely and effectively relieve moderate to severe discomfort for about 5 hours.
Some patients should avoid taking a NSAID: pregnant women, those with a history of stomach or intestinal bleeding, or heart disease (especially if following a daily low dose aspirin regimen). If you have any of these conditions or similar concerns, be sure you discuss this with your dentist before your procedure for an alternative method for pain management.
